What laws apply in international waters?

Don't quote the article from the "StraightDope" or any of its sources, I want it explained in simple terms. I understand that every ship is supposed to fly the flag of the nation it links with, but who enforces that and how do they enforce it? What if you did happen to fly a Jolly Rodger or some other flag not affiliated with a country out in international water?
